Referral Form

It is highly recommended patients bring along a referral to the consultation. 

This gives A/Prof Mark Erian a brief outline of your medical history and a reason for your visit. It is also a Medicare requirement that a referral is presented at the time of consultation, from either a General Practitioner or another specialist. 

Providing us with a valid referral will mean that you will be able to obtain a rebate from Medicare for a portion of the fee. Please click below to download a referral form.
